Share a lead form and read submissions

Open the public page of a form, send its link to people, and see what they submitted and the leads it created.

Every lead form has its own public page that anyone can open without signing in.

  1. Open CRM > Lead Forms > Forms List.
  2. Select the eye icon on the form's row. The public page opens in a new tab.
  3. Copy the address from your browser. It looks like https://app.legasum.com/forms/ followed by the form's code.
  4. Share that link by email, message, or a button on your website.

The page shows a heading, short intro text, your fields and a Submit button. After a successful submit the visitor sees "Thank you! Your response has been submitted."

When the form does not show

A visitor sees "This form is not available." if the form is not active, was deleted, or has been blocked by a platform administrator. If your company switches off the Leads module, the page may still open, but submissions are refused with the message that the form is not currently accepting submissions.

What happens to a submission

  1. The answers are saved against the form.
  2. A new lead is created with status NEW. Its Source is set to "Leadform - " followed by the form's number, and the lead's page shows an Origin note linking back to Lead Forms.
  3. If the form has a Full Name, Email or Phone field, those answers fill the lead's name, email and phone.
  4. If the email matches an existing client or lead, the lead is still created. A "Possible duplicate" note is added to the lead's Activity timeline for your team only. The visitor is never told.

The server also checks every answer: text and choices must fit their field, emails and phone numbers must look valid, and an empty submission is refused. Public submissions are rate limited to reduce spam.

Read submissions

  1. In Forms List, select the inbox icon on the form's row.
  2. Each submission shows its date and time, then every question with the answer given.

Good to know

  • Submissions are listed newest first, and there is no search box on that page. For follow-up work, use the leads they created in Leads.
  • Deleting a form also deletes its saved submissions.
